One form
One form you run: build its questions, publish and share it, read its responses, and choose what happens when one is sent.
Build the questions
- Open Build.
- Under Add, press the kind of question you want: short or long text, one choice or several, yes or no, a date, a number or a checkbox.
- Type the Question.
- Check Required when shown if it must be answered.
- Under Show this question when, choose An earlier question is answered a certain way to ask it only after a given answer.
- Choose the earlier question, and check the answers that call for this one.
- Check Sensitive for an answer that needs care.
- Press Add question.
Then: The question joins its section. Press Section under Add to start a new group of questions.
Publish it and share it
- Press Preview to see it as the people it asks will.
- Press Publish.
- Open Share.
- For a public form, press Copy beside its link, or show its QR code.
- For a unit or a team, tell them it is in My workspace › Forms.
Then: The people it asks can answer it now.
Rather not? Press Make a new link if the link has gone too far: the old link and its QR code stop opening the form at once.
After a camp, Sokha shares a public form as a QR code on the last day's slides, set to close the Sunday after and to take at most 300 answers.
Read and download the responses
- Open Responses.
- Read Results for the count of each choice.
- Press a response to open it.
- Press Reveal sensitive answers to see answers that read Hidden.
- Check Include sensitive answers (recorded) if the download needs them.
- Press Excel or CSV to download every response.
Then: Each reveal, and each download with sensitive answers, is recorded with your name and the time.
Choose what happens when a response is sent
- Open Workflow.
- Choose under Approval the role that approves each response at the form's unit.
- Choose a project under Make a task in.
- Give The task's title.
- Check the box to tell the people who run the form about each response, if you want them told.
- Press Save workflow.
Then: The cards above show what now happens: each response waits for that role, a task is made when a response is sent (or once it is approved), and whoever approves is told.
A volunteer application on the website: a public form whose health questions are marked sensitive, approved by the volunteer coordinator, with a task “Run the reference check” made in the volunteers' project for each one approved.
Set when it takes answers, and how many
- Open Settings.
- Set Opens and Closes if it should open or close by itself.
- Set Takes at most to cap the responses.
- Check or clear One response each (inside the organization).
- Choose how long to Keep responses for.
- Press Save settings.
Then: Each response is deleted that many months after it was sent.
Rather not? Press Close at the top to stop answers now, and Reopen to take them again.
Only people who hold Run forms at the form's unit open it, and a pulse survey's round only HR there, the Owner and Admins. Somebody who only approves its responses opens one response at a time, and nothing else of the form. Sensitive answers read Hidden until somebody who runs the form reveals them.
